Back-to-School Care for Kids
We offer walk-in well exams for kids, no appointments needed, through August 30, 2019.
A well-child visit covers all your child’s back-to-school health care needs. This comprehensive visit focuses on your child’s complete health—physical, developmental and behavioral. Depending on your child’s age, visits include a check of height and weight, body mass index (BMI), developmental milestones, vision, hearing and blood pressure. We’ll also review and update immunizations and discuss safety habits.
For kids in school sports, a well-child visit can also include sports clearance. Bring any sports or school-required forms to your appointment and we’ll help you complete them. If your child has been in for a well-child visit within the last year, we can help you fill out your forms.
Day, evening and weekend sports physicals are available at our HealthPartners HealthStation located in Coborn’s Superstore Sauk Rapids. Bring any sports forms with you and we’ll help you complete them.
Minnesota State High School League (MSHSL) form
Aug. 12 is the first day of practice for most fall sports. That means sports physicals need to be done by then. Bring your kids in now to ensure they can start on time.
